First Visit Preparation

Thank you for selecting us as a partner in your pet’s healthcare. We are committed to providing quality care for your pet and encourage you to call on us as the need arises. In addition to preventative healthcare and after-hours emergency consultation, we are available to assist you with any other concerns you may have regarding your pet. We understand that illness is often acute and as such, we offer same day appointments to sick patients, new or existing.

  1. New Client Information Sheet: Before your first appointment begins, you will be asked to fill out a Client Information Form so that we have updated records and contact information for both you and your pet. You may plan to fill this at the time of your appointment, or you may wish to save time and fill out a printable copy and bring it with you. If you choose to complete your paperwork at the time of your visit, please allow an additional 5-10 minutes prior to appointment time.
  2. Vaccination history/medical records: If your pet has any vaccination history, please bring that with you. Bring paperwork from the breeder or Humane Society if appropriate. If your pet has been to another veterinarian, you must request a copy of their records before your visit. For your convenience, you also may ask to have them faxed directly to us (fax number (937) 743-2603. Having proper vaccination record and patient’s history will help make the transition to our hospital smooth and will allow for optimal care for your pet. Having the medical records made available will also allow your Hunter PetCare veterinarian to become familiar with your pet, especially important if your pet has an existing medical condition.
  3. Fresh stool sample: For all puppies and kittens, we recommend bringing in a fresh stool sample(less than 12 hours old) to check for intestinal parasites. Even if your puppy or kitten has previously been de-wormed, we recommend running a fecal test as some puppies and kittens require multiple de-wormings. For adult animals, we recommend semi-annual fecal tests, consistent with The American Association of Veterinary Parasitologists recommendations. If your pet has not had a fecal checked within a year, please bring a stool sample with you. Pets can carry intestinal parasites which deprive them of valuable nutrition, comprise their overall health and pose a risk of zoonosis (ie, disease transmission from animal to humans).
  4. Bringing a cat or kitten? Check out this video for great carrier tips to help ensure a safe, pleasant and less stressful experience!

If you have any questions before your first visit, or in regards to the above information, please feel free to call our office.

The entire staff at Hunter PetCare thanks you for your patronage. We look forward to a long and healthy relationship with you and your family!
